Upload Personal Statement (no more than 1 page) describing experience, career plans, training plans (possible mentors, coursework), and plans for future funding.

K12 Proposal: 1 page describing your research plans during the K12, including: research aims (1 paragraph), how your work will focus on the learning health system, and how you will engage patients/family/caregivers.
Applicants must submit official transcripts from all undergraduate, graduate and professional schools they have attended. Transcripts should be sent directly to the T-GAPP Program from the institution to be considered official. Student copies of transcripts and diplomas will not be accepted. Three letters of recommendation are required to complete the application. One letter should be from your current division chief or chair and include commitment to a faculty position. Letters should be sent via email to gost@pennmedicine.upenn.edu
