Please complete this form by August 15th. Once completed you should receive a PDF version that you may share with your Program Advisor. This form must be completed annually, initially at the end of year 3 (prior to year 3 students must complete the CD forms). This form may be shared with MD-PhD and Grad Group leadership and associated administration.

REMINDER: This IDP will substitute for Chemistry's "Student Development Plan" that is usually submitted in Spring

Part 1: PHD PROGRESS/UPDATES AND MENTORING
1. List your main achievements in the past year (if applicable). These can include coursework, publications, fellowships, conferences, seminars, mentoring, professional development etc.
2. List your main project goals for next year.
3. What specific question is your project aimed at addressing? What are the anticipated outcomes? How does that fit in your broader field of research?
4. What skills do you need to gain in order to answer these questions?
5. Are there any anticipated challenges in achieving your research goals?
6. What additional resources do you need to complete your project? Is there anything that your advisor/mentors can do to help you achieve your objectives?
7. What is your timeline for graduation?
8. For 4th year PhD students: provide a thesis outline and list of publications that you expect to arise from your work, along with work that will be completed before you depart.

Other PhD year students: list only publications and presentations.
9. List all students (graduate, undergraduate, or high school) students who you have mentored over the past year. Are you interested in supervising undergraduate students/high school students or working with first-year graduate students?
